How do I book a routine appointment?

How do I book an urgent appointment?

How do I request a Sick (fit) Note?

If you've been off sick for more than 7 days complete our a Sick (fit) note request

If you've been off sick for 7 days or less, your employer should not ask for medical evidence that you've been ill. Instead, they can ask you to confirm that you've been ill, this can be done by filling in a form yourself when you return to work. We call this self-certification. 


When will I get by blood tests results?

If your blood test results show that you need more tests or treatment, we will contact you.
